Vocational Rehabilitation Specialist - Assertive Community Treatment (ACT)
CenClear , a provider of Assertive Community Treatment (ACT), is seeking dedicated and compassionate individuals to join the team. The ACT program is an evidence-based practice aimed at improving outcomes for individuals with severe mental illness.
ACT provides highly individualized and comprehensive services to individuals based on their needs from a holistic perspective of overall wellness. ACT recipients receive intensive multidisciplinary therapeutic, medical, and supportive services within the comfort of their own home and community. ACT team members are comprised of several domains including psychiatry, counseling, nursing, substance abuse treatment, vocational rehabilitation, housing support, case management, and peer specialist supportive services. The ACT team provides these necessary services during times that are agreed upon between team members and individuals receiving services. Hours of operation are flexible and non-traditional to meet the needs of individuals enrolled in the program. There is a 24/7 on-call components that is rotated among team members to be available for individuals when they are in most need.

Location
- This is a community-based service which includes travel within Blair, Clearfield, Elk, Huntingdon and Jefferson counties. Caseloads will be assigned in a manner to reduce as much travel as possible. The home office for the program is located in Clearfield PA, however daily presence at the office is not a requirement.
Compensation
- This is a full-time position
- 40 hours per week, 52 weeks per year (includes non-traditional hours)
- Compensation for time and mileage based on reimbursement policy
- On-call availability which is rotated among all ACT Team members.
Qualifications
- Bachelor’s degree preferably in rehabilitation science and one year of associated experience.
- Knowledge of good mental health practices and programming.
- Serves as a role model for recovery, for staff and consumers
- Strong interpersonal skills
- Ability to work collaboratively with others
- Must obtain the following clearances prior to hire: PA State Police Criminal History, PA Child Abuse History, FBI Criminal History, and Mandated Reporting Certificate
Responsibilities
- Direct Treatment and/or Support:
- Help clients access and understand benefits (SSI, Medicaid, etc.) and assist with earnings reporting
- Continuously assess clients vocational skills and needs, tailoring job searches to their unique interests
- Engage with employers to promote program support and client strengths
- Offer follow-up support to clients to help them maintain employment
- Develop individualized employment/education plans with clients, ACT team input, and family support where appropriate
- Provide education support for clients interested in furthering their education for career advancement
- Provide coaching, job coaching, and follow-up supports
- Documentation/Administration:
- Submit required documentation in a timely manner
- Maintain accurate and up-to-date medical records as per state and program regulations
- Complete time slips and documentation as required
- Document all client interactions and services in the Electronic Health Record (EHR) system
- Supervision/Training/Personal Development:
- Complete required ACT training within six months of hire
- Maintain ongoing training in ACT best practices and participate in 20 CEU hours annually
- Participate in cross-training, CenClear meetings, supervisions, and mandatory trainings
- Keep supervisors and team members informed about clients’ progress and challenges
- Support consumer choice and empowerment
- Establish trusting relationships with clients and peers
- Work independently and collaboratively as part of a team
- Adhere to all CenClear policies and procedures
- Ensure access to reliable transportation for client visits and meetings
