About The Role

We are seeking a Nurse (Near Patient Nurse) based in Poznan or nearby to support our global patient care program, TRICARE. Your responsibilities will include providing medical case management, visiting healthcare providers, writing reports, and updating medical records. This role offers a shift from clinical care to an on-site case management position, utilizing your medical expertise.

Key Responsibilities

• Provide comprehensive in-patient clinical case management following international standards.
• Maintain effective relationships with healthcare providers, stakeholders, and suppliers.
• Visit or contact care providers regularly for medical updates.
• Gather detailed clinical information from patients for accurate case management.
• Support the collection of discharge summaries and medical records from care providers.

About You

• Registered General Nurse or equivalent with at least 3 years of experience and registration in Poland.
• Recognized qualification at diploma level or equivalent.
• Committed to professional development.
• Fluent in Polish and proficient in English.
